Annual Claim Form - 2010

By now, you should have received your annual claim form. This form must be completed and returned to our office before April 30, 2010. Please click HERE to download the form if you have not received this form.

Please complete the claim form in its entirety, including the section regarding your dependents and any other insurance coverage you or your dependents may have. Be sure you and your spouse sign the form at the bottom.

Failure to complete this form in full may result in delay of payment of your claims, denial of your request for a vision claim form for you or your dependents, or denial of coverage for LASIK surgery for you or your spouse.

Health Reimbursement Arrangement (HRA) Program

The Trustees of the Chicago Laborers’ Welfare Fund are committed to providing Members with the best possible healthcare coverage.   In keeping with that commitment, we have implemented a Health Reimbursement Arrangement (HRA) program as part of your coverage under Active Plan 1, effective January 2009.   To learn more about this program, please click HERE.

Pension

Restated Plan Rules and Regulations

All pension plans are required to reapply to the Internal Revenue Service for tax-exempt status and restate our Plan Rules and Regulations, which involves submitting an updated version of the rules, including all amendments made since it was last restated on May 31, 2002. The Laborers’ Pension Fund restated our Plan Rules and Regulations on June 30, 2010. A Notice to Interested Parties was mailed between June 8, 2010 and June 19, 2010, advising of the restated rules and explaining the employees right to comment, under Section 17 of Rev. Proc. 2010-6. Annual Statement Required from Pension Recipients - 2010

With your February 2010 pension check, you will receive your Annual Statement that needs to be completed and sent back to the Fund Office by May 15, 2010.  Pensioners with direct deposit will receive a separate mailing with the annual statement form.  This form is sent every year to verify you received all of your pension checks from the prior year, to verify your mailing address, to update your file with any new information, and to verify that you are not working in any employment prohibited by the Plan rules.  A Summary About the Kinds of Work Not Allowed is also sent every year and is included with this mailing.
